I agree to place a deposit for tattoo services to be rendered at a later time. I acknowledge the deposit will be applied to the cost of the tattoo on the date of the appointment. I am aware that all deposits are non-refundable. I understand that the deposit will be lost if I no longer want the tattoo, miss my appointment for any reason or if I show up more than 15 minutes late.
In the event that you need to reschedule your tattoo appointment, a 48 hours notice prior to your appointment time and date is required. However, if you need to reschedule less than 48 hours in advance, 50% of your deposit will be forfeited as a rescheduling fee. To keep any remaining deposits or appointments scheduled, the 50% deposit forfeited must be reimbursed to the full original deposit amount by 8pm the following day or all remaining deposits and appointments will be forfeited as well.
